- How to start services
- Step One: Contact the Hotline for a phone screening. The Hotline operates 24/7/365 and can be reached at (1-800-762-0157) toll-free.
- Step Two: After contacting the Hotline, intakes are done on a first come first serve Walk In basis Monday through Friday 8am – 2pm. You must bring All required documents to complete this process.
Required Documents :- Proof of Residence
- Proof of Income
- Proof of Dependents
- Proof of Insurance
- Picture ID
- Step Three: In-person intake assessment.
- Step Four: If you are eligible for services, an appointment will be offered. If you are not eligible for services, other community resources will be offered.
