Notification of Data Security Incident

February 21, 2025 – updated December 30, 2025 – On or around December 24, 2024, Denton County MHMR Center, My Health, My Resources (“the Center”) discovered unusual activity on our network. Upon discovery, we took immediate action to secure the network and engaged a team of third-party specialists to investigate the incident. The investigation determined that certain information stored on our network was accessed by an unauthorized party for a limited time between December 24, 2024, and December 25, 2024.

 

Following a thorough analysis, on October 10, 2025, the investigation determined that potentially affected information included include patient names, addresses, patients’ identification numbers, dates of birth, diagnosis, medical history information, medical record numbers, medical treatment information, lab information, treating doctor’s name, medication information, vaccination information, medical insurance information, and biometric identifiers.

 

In response to this incident, we partnered with third-party specialists to implement additional security measures within our network and reviewed our policies and procedures related to data protection. Additionally, although we have no evidence of misuse of information as a result of this incident, we encourage potentially impacted individuals to enroll in complimentary credit monitoring and identity protection services we are making available.

Individuals are encouraged to remain vigilant against incidents of identity theft and fraud by reviewing credit reports/account statements and explanation of benefits forms for suspicious activity and to detect errors.
