Employment Assistance
This service provides assistance to an individual in locating paid, competitive employment in the community. It includes help to identify employment preferences, job skills, work requirements and conditions.
Supported Employment
This service is provided to an individual who has paid competitive employment in the community. It provides support to sustain that employment.
Specialized Therapies
This service provides assessment and treatment by licensed or certified professionals in the areas of nursing, counseling, occupational therapy, physical therapy, speech and language therapy, audiology, dietary and behavioral supports.
Vocational Training
This service provides day training in a sheltered workshop setting to enable an individual to obtain employment.
Day Habilitation
This service provides assistance with acquiring, retaining and improving self-help, socialization and adaptive skills necessary to live successfully in the community and to participate in home and community life. Day habilitation is provided in a group setting on a regularly scheduled basis and includes personal care assistance.
In-Home and Family Support
This program disburses funds to eligible individuals/families to pay for services or items that meet a need related to the individual’s disability or co-occurring physical disability. The program allows individuals to maintain or improve their ability to live in the community. Annual assistance up to $2,500 can be used to pay for specialized equipment, home modifications, medical services, personal attendant services, training and respite.
